Lentigines, “liver spots or age spots“, on the other hand are harmless, flat brown lesions on the skin that occur as a result of over exposure to the sun. They normally don’t occur until after the age of 40.

Cosmetically, both of these conditions are a nuisance. The best treatment is prevention. This includes minimizing sun exposure, applying sunscreen regularly and using a topical antioxidant. However, by the time sun-overexposure has left blotchy pigmentation, wrinkling and skin roughness, preventative measures will be ineffective. Certain cosmetic laser treatments are recommended for treating these skin imperfections.

PDT Laser Treatments for Sun Damage

Photodynamic Therapy (PDT) is an FDA-approved, safe and effective method for the treatment of a variety of skin conditions, particularly the precancerous condition of actinic keratosis and a great treatment for brown spots on the face. Therapy includes the use of a topical photosensitizing agent called Levulan, which is activated by laser light. The process is simple and virtually painless.

First, the affected area will be thoroughly cleansed and then the Levulan liquid will be topically applied. The medication is allowed to dry (incubate) for 60 – 90 minutes and following this, a laser with the correct light wavelength is used to activate the Levulan. After this activation, the treatment area is cleansed again and sun block is applied. Sun avoidance is mandatory for 24 hours after treatment. The process of PDT results in the destruction of abnormal skin cells (AK’s) decreased skin redness and reduction of brownish skin discolorations.

Laser Resurfacing Sun Damaged Skin

Because the wavelength of light of Erbium Laser is not as long as that of CO2 and because there is more control during treatment, Erbium laser skin resurfacing is much safer, has less downtime and offers pain-free recovery as compared to resurfacing with the CO2 laser. Furthermore, by using the Erbium laser in a fractionated mode, there is less post-procedure swelling and redness and the recovery time is quickened, usually being only 24 hours. Of equal importance, for those having darker skin tones, the erbium laser poses much less risk of skin lightening or hypopigmentation.

Low level Laser Light Therapy (LLLT) is among the newer hair loss treatments being used today and we at the The Randali Centre have been so impressed with its effectiveness that we are now offering this option to our patients.

This past Monday, Casey received her third (and final) Sculptra treatment. Results from her Sculptra treatments should last beyond two years. The treatment is a series of non-surgical procedures over a period of several months, where lost collagen in the skin is regenerated and renewed.

Casey spoke about the details of the procedure on air:

She does no cutting at all – this is all non-surgical… Afterwards, you might (and that’s might) see a little bit of bruising. I did last time right around my mouth, but it wasn’t anything that I couldn’t cover up with makeup.

Casey’s co-host of the WIOV morning show, Murf, said that he was hesitant at the beginning of the course of various treatments that comprised Casey’s transformation:

I was a little bit skeptical when this whole first thing came about. Because… I didn’t think Casey needed anything to look more naturally radiant than she is… but then she did meet up with Dr. Cohen, and I’m here to verify that the results have been fabulous, to say the least.

Dr. Cohen has also helped to get rid of Casey’s sun spots. In addition to this, Casey has been using Latisse for several weeks. She laughed and said, “I’ve also been complemented about how I bat my long eyelashes at people lately.”

Casey said that Dr. Cohen “takes away those wrinkles and makes you feel younger. She stays on top of the newest technology.”

Casey now has one more Restylane treatment planned for the under-eye area. Also, stay tuned for the “after” photos, which should be posted soon!

Casey will receive a third and final treatment of Sculptra in early April. The results are expected to last well beyond two years. Casey is also continuing the use of the Obagi NuDerm skin care system, which is only available through a physician like Dr. Cohen.

Dr. Cohen recently also augmented Casey’s lips with Restylane, and the results were great. “I never thought I wanted to do a lip enhancement,” Casey said. “Dr. Cohen said your bottom lip should be just a tiny bit bigger than your top lip, and mine wasn’t. So she did that, and it’s very subtle.”

Casey also had an appointment with cosmetic dentist Dr. John Weierbach to follow up to the teeth whitening he had performed earlier along with straightening her front lower teeth. Dr. Weierbach is placing permanent crowns on some of Casey’s teeth and building out the side of some teeth to give Casey a perfect, straight smile.

About receiving treatment from Dr. Cohen, Casey said, “I’ve never dealt with any doctor who has been more caring and concerned about their patients. She deals with everybody on an individual basis. I believe in her, and she has proven to me that she’s up-to-date on everything that she has to know and to do with people’s skin.”

My face is a little bit red from the new system, a little bit flaky, but that’s about all. But you know what? If that didn’t happen, then I wouldn’t think it was working. So I’m very glad for this reaction, which will all go away. You can’t even tell, because I can easily cover it with makeup.

A couple days later, she saw that the flaking was going away and her skin was being rejuvenated. She said, “My skin looks so much clearer, and my pores aren’t as big anymore. You should see how good my skin looks. I absolutely love this skin care system.”

Also since our last update in January, Casey has finished a course of five Accent Thermotherapy skin tightening treatments around the eye area. With those treatments combined with the Obagi system, she noticed that it was becoming easier to put on eye makeup:

Ladies, you’re going to back me up on this – as we get older, at your eyes, right below your eyebrows, that starts to fall down. And mine was covering my eyelid. So when I would put on my eye makeup, you couldn’t even see it. When I would put my mascara on, I used to have to grab ahold of my eyebrow and pull it up, because my eyelashes would touch up there. Suddenly I’m noticing I don’t have to hold up my eyebrow anymore, because my face has been lifted. And it’s not any surgery!

Dr. Cohen also treated Casey once with a Sculptra facial volumization (what some call a nonsurgical “liquid facelift”) and Radiesse chin augmentation.

 As all of us age, we lose collage, and that’s what gives your face that volume and keeps the wrinkles away. But as everyone ages, you lose that, because you lose the collagen. Not only that  –Dr. Cohen told me that you lose bone mass, too. Right around my mouth is where I was really starting to see wrinkles come in. Radiesse is a filler that immediately takes effect, and the cool thing is that it also stimulates your collagen so that this is going to last you at least a year. This doesn’t change the way you look, it just enhances the way you look.
